To make your donation, you must click "RETURN TO FORM" which is towards the bottom of the verification form and you will be returned to this page.  To make your donation you will need to go to the bottom of this page and click "Credit Card" if you intend on making your donation via credit card or click "Cash or Check" and complete that form.
